Job summary

Pathways Healthcare is seeking a Nurse Practitioner to provide in-home medical care as part of a physician- and NP-led team. You will conduct admissions, develop personalized plans and deliver routine and urgent visits in Worcester, Sturbridge and surrounding areas.

The role requires an APRN license (Adult/Geriatrics preferred), DEA and CPR, a Master’s in APN, and at least 2 years in home health. A valid driver’s license and reliable transportation are essential.

Qualifications

• APRN license, DEA certificate and CPR certification are required.
• Master’s degree in Advanced Practice Nursing from an accredited school.
• At least 2 years as a Nurse Practitioner, preferably in home health care.
• Valid driver’s license with reliable transportation.
• Excellent communication, problem‑solving and organizational skills; Homecare Homebase knowledge is a plus.

Responsibilities

• Conduct admission assessments and develop personalized treatment plans.
• Provide routine and urgent care visits with flexibility to meet patient needs.
• Manage medication orders and educate patients on proper usage.
• Handle wound care, testing and equipment/supply needs.
• Document patient progress and collaborate with the care team for optimal results.
• Assist in discharge planning and coordinate with community resources.
• Participate in team meetings and briefings to ensure cohesive patient care.
• Maintain patient confidentiality and report any incidents or safety concerns.
