Overview
Interested in joining a nationally recognized, Value Based Care organization that focuses on providing secondary preventative care for patients with chronic kidney disease and end stage renal disease? This organization offers a strong work/life balance with no weekends or on-call required and more time with patients. Our client seeks a Nurse Practitioner to join their growing team around Elk Grove, CA. The organization partners with nephrologists throughout California. Nurse Practitioners provide hands-on case management and clinical education to prevent hospitalizations and the need for dialysis.
Compensation
Base pay range: $145,000/yr - $160,000/yr. Final pay is based on skills and experience.
Responsibilities
- Travel to local nephrology clinics 3-4 days per week.
- Perform administrative work and provide telehealth services from home 1-2 days per week.
- Perform comprehensive physical evaluations.
- Develop individualized care plans and ensure appropriate follow up through the care continuum.
- Work collaboratively with an interdisciplinary team including nephrologists, RNs, dietitians, cardiologists, etc.
- Provide coordinated care services between the patient, family, physicians and other care team members.
Qualifications
- Master’s Degree in Nursing
- Licensed Nurse Practitioner in the state of California
- 1+ year of experience as a Nurse Practitioner within Nephrology, Cardiology, Primary Care or Internal Medicine
- Current DEA or eligible to obtain within 90 days of hire
- Current driver’s license and insurance within California

- Schedule: Monday–Friday, flexible start/end time; 3-4 in-clinic days per week and 1-2 telehealth/work-from-home days
