Nurse Practitioner HouseCalls Mid-West Regional Traveler - IA KS MN MO ND NE SD WI

Optum is seeking a Midwest Regional Traveler Nurse Practitioner to join our HouseCalls team, covering KS, MO, IA, MN, NE, ND, SD, and WI. This role involves providing annual clinical assessments in patients' homes to promote preventive care and coordinate follow-up care, helping to improve health outcomes.

Enjoy flexible scheduling with no on-call, weekends, or holidays required, and the role involves approximately 85% travel across the Midwest. All travel expenses are covered, and compensation includes a competitive base salary plus incentives.

Primary Responsibilities

• Conduct in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
• Perform screenings and point of care testing as appropriate.
• Identify diagnoses for care management and active treatment.
• Communicate findings to the patient's PCP and identify urgent situations.
• Educate patients on health topics such as disease management and medication adherence.

Qualifications

• Active NP license or ability to obtain one by start date.
• National certification in relevant fields or ability to obtain it.
• Ability to perform physical assessments and navigate various home environments.
• Reliable transportation for extensive travel.

Preferred Qualifications

• Experience in geriatrics, cardiology, internal medicine, or endocrinology.
• Home health or home visit experience.

The salary range is $89,800 to $176,700 annually, based on full-time employment and various factors. Benefits include comprehensive packages, incentives, and 401k options.
