Position Overview

Optum is seeking a Midwest Regional Traveler Nurse Practitioner to join our HouseCalls team, covering KS, MO, IA, MN, NE, ND, SD, and WI. This role involves providing annual clinical assessments in patients' homes, focusing on preventive care to identify health risks and coordinate follow-up care. The position offers flexible scheduling, no on-call, weekends, or holidays, and requires travel across multiple states, with expenses covered and a competitive salary plus incentives.

Primary Responsibilities
  1. Conduct in-home assessments, including medical history review, physical exams, and vital signs.
  2. Perform screenings and point-of-care testing as appropriate.
  3. Identify diagnoses and formulate care management plans.
  4. Communicate findings to primary care providers and identify urgent situations.
  5. Educate patients on health topics and medication adherence.
Qualifications
  • Active NP license or ability to obtain one by start date, with no disciplinary actions.
  • National certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care or Acute Care.
  • Physical ability to perform assessments and lift up to 30 pounds.
  • Reliable transportation for travel up to 85% within licensed states.
Preferred Qualifications
  • Additional licensure in other states.
  • Over 1 year of clinical experience.
  • Experience in geriatrics, cardiology, internal medicine, or endocrinology.
  • Home health or home visit experience.
Compensation and Benefits
