Nurse Practitioner HouseCalls Mid-West Regional Traveler - IA KS MN MO ND NE SD WI

Optum is seeking a Midwest Regional Traveler Nurse Practitioner to join our HouseCalls team, covering KS, MO, IA, MN, NE, ND, SD, and WI. This role involves providing annual clinical assessments to patients in their homes, promoting preventive care and health risk reduction.

Enjoy flexible scheduling with no on-call, weekends, or holidays. The role requires travel across multiple states, with all expenses covered, and offers a competitive salary plus incentives.

Responsibilities

• Conduct in-home assessments including medical history, review of systems, medication reconciliation, vital signs, and physical exams.
• Perform screenings and point of care testing as appropriate.
• Identify diagnoses for care management and active treatment.
• Communicate findings to inform the patient's PCP and coordinate follow-up care.
• Identify urgent situations and intervene accordingly.
• Educate patients on disease management, medications, and compliance.

Qualifications

• Active NP license or ability to obtain by start date, free of disciplinary actions.
• Certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care or Acute Care.
• Ability to perform physical tasks such as lifting, navigating stairs, and physical assessments.
• Reliable transportation for up to 85% travel within states.

Preferred

• Additional licensure in other states.
• 1+ years of clinical experience.
• Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
• Home health or home visit experience.

Salary ranges from $89,800 to $176,700 annually, based on various factors. Benefits include comprehensive packages, incentives, stock purchase, and 401k.

Employees must commit to at least 12 months in the role and 36 months to fully qualify for student loan repayment benefits.
