About the role

$40,000 Student Loan Repayment or $30,000 Sign-on Bonus for individuals who have not previously participated in this program.

Optum is seeking a Midwest Regional Traveler Nurse Practitioner to join our HouseCalls team, covering KS, MO, IA, MN, NE, ND, SD, and WI. This clinician-led care organization aims to create a seamless health journey for patients across the care continuum.

As part of the Home and Community Care team, you’ll conduct annual clinical assessments in patients’ homes, providing preventive care to identify and reduce health risks, and coordinate follow-up care to improve health and well-being.

Our holistic approach addresses physical, mental, and social needs, helping patients access and navigate care anytime and anywhere, creating a seamless health journey across settings.

This role offers work-life balance with flexible scheduling over a 7-day week, no on-call, weekends, or holidays required. It involves approximately 85% multi-state travel within the Midwest, with all travel expenses covered, and includes a competitive salary plus incentives.

Primary Responsibilities
  1. Conduct in-home assessments: medical history, review of systems, medication reconciliation, vital signs, physical exam
  2. Perform screenings and point of care testing as appropriate
  3. Identify diagnoses for care management and treatment
  4. Formulate a list of medical conditions based on assessments
  5. Communicate findings to inform the patient’s PCP
  6. Identify urgent/emergent situations and intervene
  7. Educate members on disease, medication, and compliance
Minimum Qualifications
  • Active NP license or ability to obtain by start date, free of disciplinary actions
  • Autonomous practice approval if applicable, or ability to obtain it within specified timeframes
  • Active national certification in Family, Geriatrics, Adult, or Adult-Gerontology primary care or acute care
  • Physical ability to perform assessments, lift 30 pounds, navigate stairs, sit, stand, kneel
  • Reliable transportation for up to 85% travel within licensed states
Preferred Qualifications
  • Additional state licensure
  • 1+ years of clinical experience
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
  • Home health or home visit experience

The salary range is $89,800 to $176,700 annually, depending on experience and location. Benefits include comprehensive packages, incentives, stock purchase, and 401k. Employees must remain in the role for at least 12 months, and full benefits apply after 36 months in the role.
