Job summary

Del Corazon is seeking a Hospice Nurse Practitioner (NP) in Los Alamos, NM to conduct face-to-face visits with patients in homes or facilities and collaborate with the Medical Director to ensure comprehensive patient care.

The role requires an NP with NM licensure, 1+ year hospice/palliative care experience, and APRN certification. Responsibilities include symptom management, care plan adjustments, documentation, and education for families and caregivers.

Qualifications

• Master’s or Doctorate in Nursing required and NP licensure in NM.
• Minimum of 1 year experience as an NP in hospice or palliative care.
• Current APRN certification.
• Strong clinical assessment and communication skills with an empathetic approach.

Responsibilities

We are seeking a skilled NP to conduct face‑to‑face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.

Key Responsibilities

• Conduct face‑to‑face visits with hospice patients in their homes or facilities to assess and support their clinical needs
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
• Document patient encounters accurately and timely to ensure regulatory compliance
• Serve as a resource for clinical questions and assist in educating families and caregivers about the end‑of‑life process and available resources

Qualifications

• Education: Master’s degree or Doctorate in Nursing, with current NP licensure in New Mexico
• Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting
• Certification: Current Advanced Practice Registered Nurse (APRN) certification
• Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
