Job summary

Preferred Home Health Care & Nursing Services is seeking a Hospice Nurse Practitioner to provide palliative and symptom-management care to hospice patients in collaboration with an interdisciplinary team.

The ideal candidate holds an unrestricted NP license, national certification, CPR, and at least 5 years of nursing experience, with reliable transportation and a patient-centered approach to support families through end-of-life care.

Qualifications

• Unrestricted active license and current certification as a Nurse Practitioner or Clinical Nurse Specialist.
• Certification from a recognized national body (ANCC, NBCHPN, AANP).
• CPR certification and minimum of 5 years nursing experience.
• Valid driver’s license, access to transportation or public transit.

Responsibilities

• Conduct face-to-face visits with hospice patients, reporting findings to the Hospice Medical Director.
• Collaborate with physicians to develop and coordinate individualized care plans.
• Document patient conditions, goals, interventions, and responses, ensuring accurate clinical notes.
• Educate and supervise patients and families in self-care techniques when appropriate.
• Communicate patient needs and issues to the healthcare team and administrative staff.
