Job summary

Jobtailor is seeking a licensed Nurse Practitioner (APRN/NP) to join our hospice care team. The role emphasizes direct patient encounters, symptom management, and development of individualized Plans of Care in collaboration with physicians and IDT members.

Responsibilities include documenting findings, supporting the Medical Director, attending IDT meetings, and ensuring continuity of care across patient visits. Active NP license and two years of clinical nursing experience are required.

Qualifications

• Active APRN/NP license is required.
• Minimum of two years of clinical nursing experience.
• Graduate of an approved Nurse Practitioner curriculum.

Job description

• Conduct face-to-face encounters for patients in their third or subsequent benefit period
• Document and promptly share findings with the certifying physician
• Conduct well-patient checks
• Conduct symptom management visits
• Provide physician support
• Coordinate and collaborate with IDT members, community physicians, facility staff, and others as needed
• Participate in developing Plans of Care for assigned patients
• Enhance care strategies with the Hospice Clinical Manager and Medical Director
• Serve as a resource for Hospice staff to ensure continuity and improve patient care quality
• Attend IDT meetings
• Provide backup support to the Hospice Medical Director and cover medical needs of hospice patients as required

Requirements

• Graduate of an approved Nurse Practitioner curriculum
• Active APRN or NP license required
• Minimum of two years clinical nursing experience
• Valid driver’s license
