Daymark Health in Lansing, MI is seeking a qualified Nurse Practitioner or PA to join our home-based cancer care team. You will deliver hands-on clinical care in patients’ homes and via virtual visits, while helping to refine our care model and processes.
You will collaborate with our operations team, triage high‑acuity cases, and support patients through active treatment, survivorship, and palliative care, ensuring compassionate, evidence‑based care at every step.

As one of Daymark’s Nurse Practitioners, you’ll work with our care team to implement our cancer care model with patients in their homes. Your primary goal is to provide hands‑on, compassionate patient care directly in patients’ homes and through virtual platforms. You’ll also implement and help to continuously improve processes that will enable Daymark to be effective in Lansing, build a highly engaged patient population, and contribute to a high‑performing clinical team. You must be comfortable operating as a player/coach, rolling up your sleeves, and doing whatever is required to provide high‑quality clinical care and to grow quickly. You’ll work closely with our clinical operations team, leveraging your expertise to establish our evidence‑based oncology care and navigation model to address clinical (symptom management, in‑home urgent care, palliative care, mental and behavioral health care) and non‑clinical (social and related needs).
The expected compensation for this role is $140,000 to $170,000 annually, depending on location & experience.
This is a full time role operating on a 4 day / 10 hour schedule, Mon - Fri.
All Daymark Health patient‑facing roles require Basic Life Support (BLS) certification and compliance with Daymark's Employee Vaccination and Testing policy.
For roles that require in‑home visits to patients, candidates must have a reliable personal vehicle and provide proof of a valid driver's license.
