As a Landmark Health Provider, now part of Optum, you will visit patients at home, spend adequate time to address health issues, build trusting relationships, and work with a multidisciplinary team—all at no cost to the patient.

Connecticut Residents Only: Salary ranges from $85,000 to $167,300, depending on various factors. Benefits include comprehensive packages, incentive programs, stock purchase, and 401k.
