$40,000 Student Loan Repayment or $20,000 Sign-on Bonus for individuals who have not previously participated in this program.

As a Landmark Health Provider, now part of Optum, you will visit patients at home, spend adequate time to address health issues, build trusting relationships, and work with a multidisciplinary team—all at no cost to the patient.

Primary Responsibilities:


  • Manage chronic illnesses in elderly, multimorbid patients, focusing on wellness and avoiding unnecessary hospitalizations and ER visits.
  • Document thoroughly, focusing on closing quality gaps and recapturing HCCs.
  • Conduct comprehensive geriatric assessments for new patients, including functional, social, safety, cognitive, and mental health evaluations.
  • Triage, diagnose, and treat patients at home using available equipment and medications.
  • Order and interpret tests such as x-rays, labs, and EKGs, and prescribe treatments within scope of practice.
  • Initiate treatment at the patient's home.
  • Collaborate with the patient's PCP and specialists as needed.

Qualifications - External

Required Qualifications:

  • Completion of an accredited Nurse Practitioner (NP) or Physician Assistant (PA-C) program.
  • Current unrestricted NP or PA license in your state.
  • Valid state driver's license and reliable transportation.
  • Full COVID-19 vaccination, including boosters if mandated by state regulations.

Preferred Qualifications:

  • 2+ years clinical experience as NP or PA in IM, Geriatrics, Primary Care, ED, Urgent Care, or similar.

All employees must disclose COVID-19 vaccination status and comply with applicable regulations. Failure to meet vaccination requirements may lead to job offer rescission or termination.

Connecticut Residents Only: Salary ranges from $85,000 to $167,300, depending on various factors. Benefits include comprehensive packages, incentive programs, stock purchase, and 401k.

Note: Employees must be active in role for at least 12 months; student loan benefits require a 36-month role commitment within Home and Community.
