Job summary

U.S. Department of Veterans Affairs in the Home Based Primary Care program seeks a Nurse Practitioner to provide comprehensive, patient-centered care to Veterans in a home-based setting.

Qualifications

• English proficiency required for direct patient care.
• Graduate of a nursing program accredited by ACEN/CCNE.
• State license in nursing required; bridge program options available.

Responsibilities

• Independently manage a panel of HBPC patients with acute and chronic conditions.
• Develop and implement individualized care plans.
• Collaborate with interdisciplinary team and provide education to patients.
