Job summary

The Home Based Primary Care Nurse NP at the Veterans Health Administration provides independent practice in Community Outpatient Services (COPS) / Home Based Primary Care (HBPC) programs to deliver preventive and chronic care with advanced assessment and evidence-based protocols for patients in Akron, OH.

This role emphasizes health promotion, disease prevention, and coordination within an interdisciplinary team, including ordering tests and coordinating referrals, while upholding ICARE values

• Prescribes pharmacologic and non-pharmacologic interventions

Summary

The Home Based Primary Care Nurse NP is a professional - independent practitioner with advanced education and appropriate experience who provides evidence-based care for a specific population of patients. The NP uses advanced assessment - planning and evaluation - as well as approved standardized procedures - to provide preventive and illness management healthcare incorporating the patient's age - gender - culture and learning abilities in the treatment plan.

Duties

• The Advanced Practice Nurse (APN) II Certified Nurse Practitioner (NP) practices in Community Outpatient Services (COPS)/Home Based Primary Care (HBPC) Program to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
• The NP provides advanced clinical practice - consultation - and management
• The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
