Do you have two available weeks per month and looking for a flexible, autonomous PRN opportunity to supplement your income while making a meaningful impact?

We are seeking Nurse Practitioners (NPs) willing to travel regionally to conduct in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Must be licensed in at least three of the following states; North Carolina, South Carolina, Texas, Illinois, Ohio, Michigan, Minnesota, Maryland, Georgia, Kansas, Wyoming, Idaho, New York, or Montana.

Position Responsibilities

As a Nurse Practitioner you will:

???? Conduct comprehensive in-home Health Risk Assessments (HRAs) for Medicare Advantage members.

Review medical history, medications, lifestyle, and social determinants of health.

Perform physical and mental health evaluations and document findings in real-time.

???? Provide preventive care education without the need for ongoing patient management.

Travel regionally for in-home assessments.

Qualifications

️Active Nurse Practitioner(NP) (all specialties except PMHNP, CRNP)

️ Minimum of 2 weeks per month including 5 consecutive days for a period of six months.

️ Proprietary EMR System – Simple,streamlined documentation via a company-provided iPad.
