Job summary

Epic Healthcare in Cranford, NJ seeks a compassionate Nurse Practitioner to provide primary and ongoing care to residents in a skilled nursing facility. The NP will collaborate with physicians, nurses, and the interdisciplinary team to ensure high-quality medical care for residents.

The ideal candidate holds an active NJ NP license, is board-certified or pursuing certification in FNP/AGNP/GNP, and has experience in SNF/geriatrics.

Qualifications

• Active New Jersey NP license required.
• Board certification preferred (FNP, AGNP, or GNP).
• Experience in skilled nursing facility, long-term care, or geriatrics preferred.
• Strong clinical decision-making and communication skills.

Responsibilities

• Conduct routine and acute patient assessments for nursing home residents.
• Manage chronic conditions and develop treatment plans.
• Respond to changes in residents' conditions and provide timely care.
• Order and interpret diagnostic tests and labs.
• Prescribe medications in accordance with New Jersey regulations.
• Coordinate care with physicians, nursing staff, and families.
• Ensure proper documentation in the EMR.
• Participate in care plan meetings and regulatory compliance.
