U.S. Department of Veterans Affairs in Tomah, WI seeks a Certified Nurse Practitioner for the Community Living Center/Extended Care to provide advanced clinical care with emphasis on quality and timely service, while advancing VA mission.

The NP will lead clinical practice, consult and manage programs, and serve as an operational resource for staff within Medicine Service/Geriatrics and Extended Care. Telework is not available.

• Independently manages a panel of patients with acute and chronic health conditions.
• Develops and implements an individualized plan of care to achieve optimal outcomes.
• Provides education to patients and their families.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
