Job summary

EPIC Healthcare in New Jersey is seeking a compassionate Nurse Practitioner to provide primary and ongoing care to residents in a skilled nursing facility. The NP will collaborate with physicians, nurses, and the interdisciplinary team to ensure high-quality care.

Responsibilities include routine and acute assessments, managing chronic conditions, coordinating with families and staff, ordering tests, documenting in the EMR, and contributing to care plans and regulatory compliance.

Qualifications

• Active New Jersey Nurse Practitioner license.
• Board Certified NP preferred (FNP/AGNP/GNP).
• Experience in SNF/long-term care or geriatrics preferred.
• Strong clinical decision-making and communication skills.
• Ability to collaborate with nursing home staff.

Responsibilities

• Conduct routine and acute assessments for nursing home residents.
• Develop and manage treatment plans for chronic conditions.
• Coordinate care with physicians, nursing staff, and families.
• Order and interpret diagnostic tests and labs.
• Ensure proper documentation in the EMR.
• Participate in care plan meetings and regulatory compliance.
