Job summary

EPIC Healthcare in New Jersey is seeking a compassionate Nurse Practitioner to provide primary and ongoing care to residents in a skilled nursing facility. The NP will collaborate with physicians, nurses, and the interdisciplinary team to ensure high-quality care.

Responsibilities include routine and acute assessments, managing chronic conditions, coordinating with families and staff, ordering tests, documenting in the EMR, and contributing to care plans and regulatory compliance.

Qualifications

• Active New Jersey Nurse Practitioner license.
• Board Certified NP preferred (FNP/AGNP/GNP).
• Experience in SNF/long-term care or geriatrics preferred.
• Strong clinical decision-making and communication skills.
• Ability to collaborate with nursing home staff.

Job Overview:

We are looking for a compassionate and experienced Nurse Practitioner to provide primary and ongoing care to residents in a skilled nursing facility. The NP will work closely with physicians, nurses, and the interdisciplinary team to ensure residents receive high-quality medical care.

Responsibilities:

• Conduct routine and acute patient assessments for nursing home residents

• Respond to changes in residents’ conditions and provide timely care

• Order and interpret diagnostic tests and labs

• Prescribe medications in accordance with New Jersey regulations

• Coordinate care with physicians, nursing staff, and families

• Ensure proper documentation in the EMR

• Participate in care plan meetings and regulatory compliance

Requirements:
