Guidestar Eldercare is seeking a Nurse Practitioner to address the needs of aging populations with dementia in nursing homes and assisted living facilities. You will practice with autonomy, supported by corporate clinical leadership and in-house scheduling, coding, and billing.

As part of a multidisciplinary team, you will travel daily to facilities, perform initial evaluations, cognitive testing, and ongoing therapy services, and document care needs while building strong relationships with

Qualifications

• Proof of Minnesota state licensure for NP.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.

Responsibilities

• Travel daily to assigned facilities from your home.
• Provide initial evaluations, continuing therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with client facilities, patients, and their families.
