We Care Health Centers in Ohio seeks a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities, manage acute changes, and participate in care planning with an interdisciplinary team.

Candidates must hold OH NP license, board certification, and DEA; LTC experience and EMR proficiency preferred. Competitive benefits include 401(k), insurance options, paid time off, and a $5000 sign-on bonus.

Qualifications

• Current, unrestricted NP license in OH.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.

Responsabilités

• Provide primary and preventative care for long term care patients in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per OH law.
• Order, review and follow-up with treatments and diagnostic testing.
• Communicate with nursing home staff on changes in condition and care plans.
• Collaborate with primary care physician.
