Responsibilities

• Provide assessment, diagnosis, and management of acute and chronic conditions.
• Conduct regular patient rounding and timely evaluations.

• Prescribe and manage medications in compliance with regulations.
• Maintain accurate, timely documentation in the EMR.
• Collaborate with nursing, rehab, pharmacy, hospice, and administrative teams.
• Respond to change-in-condition and urgent care needs.
• Support advance care planning and end-of-life discussions.
• Participate in quality assurance and regulatory readiness efforts.
