Nurse Practitioner – Contract Opportunity
Locations: Monroeville, Cheswick, McKeesport, Pittsburgh (Allegheny County, PA)
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Contract Length: 7 Months
Start Date: Immediate Hire (Virtual Interviews Available)

Key Responsibilities

Complete one-time, comprehensive patient assessments

Perform physicals, medication reconciliation, cognitive and nutritional screenings

Develop and document treatment plans and problem lists

Facilitate goals-of-care discussions (training provided)

Collaborate with PCPs, facility staff, and family members

Ensure safe transitions of care between facilities and home

Ideal Candidate Profile

Licensure: Active Pennsylvania NP license

Experience: 2+ years in geriatrics, primary care, LTC, or SNF settings

Skills:

Proficient in clinical evaluations and patient management

Strong communication and collaboration abilities

Willingness to travel to multiple facilities within the county

Vaccination: COVID-19 vaccination required
