Our team of physicians and nurse practitioners collaborates with assisted living, nursing, independent living, or long-term care facilities to ensure that our patients receive top-notch medical care without the challenges of traveling to a doctor's office. As we continue to expand, we are seeking dynamic and compassionate nurse practitioners to join us in providing specialized care to older adult patients throughout the communities that we serve.
RESPONSIBILITIES:
- PATIENT ASSESSMENT AND EVALUATION:
- Conduct through health assessments to identify patient need and develop personalized care plans.
- Regularly evaluate patient conditions and adjust treatment plans as necessary.
- CLINICAL CARE:
- Administer medications, treatments, and other therapeutic interventions as prescribed.
- Perform minor procedures and wound care as required.
- Monitor vital signs and patient response to treatment.
- PATIENT EDUCATION:
- Educate patients and their families on health management, disease prevention, and treatment options.
- Provide guidance on lifestyle modifications and self-care practices.
- COLLABORATION AND COMMUNICATION
- Work closely with physicians, nurses, and other healthcare professionals to coordinate comprehensive patient care.
- Maintain clear and concise communication with interdisciplinary team members.
- DOCUMENTATION:
- Accurately document patient assessments, care plans, and interventions in a timely manner.
- Ensure compliance with all relevant healthcare regulations and standards.
QUALIFICATIONS:
- EDUCATION AND LICENSURE
- Master’s degree in nursing with a specialization as a Nurse Practitioner in General Medicine or a related field of practice.
- Current and unrestricted Ohio state licensure as a Nurse Practitioner.
- Active and valid DEA license.
- Ability to participate in Medicare and Medicaid programs.
- EXPERIENCE
- Proven experience as a Nurse Practitioner, preferably in-home, in a Skilled Nursing Facility, Assisted Living, or a related area of expertise.
- Familiarity with geriatric and chronic care management.
