Job summary

Penn Medicine is seeking a Diabetes Nurse Practitioner to join our outpatient Endocrinology and Diabetes team at University City. You will work with physicians, APPs, nursing, nutrition, and pharmacy to improve diabetes care across UPHS, establishing follow-up for new patients.

The ideal candidate will have NP credentials, acute care certification, and be available Monday through Friday, 8 a.m. to 5 p.m., contributing to a multi-disciplinary team and ongoing quality initiatives.

Qualifications

  • CRNP with active NP license and credentialing.
  • Acute care certification required for clinical practice.
  • Experience as an NP in diabetes/endocrinology is preferred.

Responsibilities

  • Provide direct clinical care for diabetes patients in outpatient setting.
  • Document evaluations, diagnoses, treatments and outcomes in EHR.
  • Collaborate with physicians, PAs, nursing, nutrition, and other teams.
  • Establish follow-up care for new diabetes patients.
  • Support accreditation, compliance and regulatory activities.
  • Precept NP students and other health care learners.

Summary:

The Diabetes Nurse Practitioner functions as part of a team that consults and manages Patients with diabetes in our outpatient practice at Penn Endocrinology and Diabetes University City.
Work in collaboration with hospital staff including Physicians, Advanced Practice Providers, Nursing,, Nutrition, Information Systems, Pharmacy, and CEQI as members of a multi-disciplinary team to improve the care of patients with diabetes across UPHS. Establishes follow up care for patients new to diabetes. The ideal candidate will be available Monday - Friday to work 8-hour shifts; 8a-5p.

Responsibilities:

  • Provides direct clinical care for patient population according to established protocols and utilizes evidence based practice standards in collaboration with team.
  • Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR).
  • Identifies self properly to the patient and family, and clarifies the role of the nurse practitioner if necessary.
  • The nurse practitioner should function within their scope and refer to the collaborating physician when outside of scope, and/or directed by the collaborating physician or when requested by the patient and/or family.
  • Performs interventions specific to assigned patient population and collaborative agreement.
  • Participates in the performance improvement processes to meet quality of care and service standards.
  • Demonstrates awareness of professional responsibility as role model to peers and members of the health care team.
  • Perform approved therapeutic or diagnostic procedures based upon patient’s clinical status.
  • Addresses patient and family concerns and creates a therapeutic relationship to provide support.
  • Provide patient education, counseling and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.
  • Participate in and support accreditation, compliance and regulatory activities of the organization.
  • Precept NP students and other health care learners.
  • Identifies patient/family needs for community resources and refers appropriately.

Credentials:

  • Basic Cardiac Life Support (Required)
  • BLS/CPR, as a healthcare provider as per the American Heart Association (For providers in the ambulatory setting) required.
  • Certified Nurse Practitioner PA (Required)
  • Prescriptive Authority (Required)

Education or Equivalent Experience:

  • Master of Arts or Science (Required)
  • Education Specialization: Master's Degree or Doctor of Nursing Practice (DNP) in Nursing Equivalent Experience: •And New to practice or < 2.5 years, The CRNP must be reviewed by the Credentials Committee and approved by the Medical Board.
  • Acute Care Certification (Required)
  • Adult/Adult Gero NP certification, Adult Primary Certifications (preferred)
  • Family NP Certification Accepted
