Job DetailsJob Location: HARLINGEN CLINIC - HARLINGEN, TX 78550

GENERAL DESCRIPTION OF POSITION:

Performs all duties to ensure the patient has a pleasant and satisfactory experience in the way their health care is delivered, managed, coordinated with both internal and external entities and in which the patient participates in decision making. As a member of the clinical team, this position requires total support of the PCMH principles, policies, and procedures as appropriate, provider will be providing services to patients by any of the following methods: telemedicine, telehealth, telephonic.

Functions under staff physicians in the provision of medical care and supporting a panel of patients assigned to physician. Work involves assessing, planning, and providing patient care. When assessing the patient, the age of the patient is taken into consideration. As a member of the clinical team, this position requires total support to the PCMH principles, policies, and procedures. This position will be headquartered in Harlingen with expectation to travel to Santa Rosa, Raymondville and Brownsville anytime that there is a provider vacancy.

TRAVEL REQUIRED: 50%

ESSENTIAL JOB FUNCTIONS:
  • Supports pediatric and adult patient care. Initiates brief basic diabetes and nutrition education for newly diagnosed diabetics and hypertensive patients. Participate in clinical team meetings for identification of high-risk super utilizers. Participates in daily huddles. Assess walk in patients that nursing triage determines needs to be seen by provider, provide care with disposition or consult with physician. Assists with reviewing charts and completing DME and other forms that are necessary and follows up on referrals as needed.
  • Assures all pre-visit work is completed by MA team to include pertinent hospital discharge data. Assures standing orders for health maintenance are completed. Performs annual wellness visit for Medicare ACO (Accountable Care Organization), THSteps with follow up appointment with physician scheduled as needed. Reviews qualify performance for identified panels and develop corrective actions or care plans with team to reduce unnecessary utilization of ER or Hospital. Answers critical patient phone calls that have been referred to provider by nurse message line. Works at the highest level of credentials, delegating other duties as necessary.
  • Obtains a health and development history from patient or patient’s parent and records findings in a succinct, systematic, accurate, problem-oriented manner and critically evaluates the history. Evaluates health care needs of patients and develops plans to meet these needs.
  • Performs a basic physical assessment using techniques of observation, inspection, auscultation, percussion, and palpation. Performs or requests special screening and development tests, lab tests and interprets results. Provides health care and preventive service to healthy individuals, including guidance in nutrition, episodic illnesses, child growth and development, problems of child rearing, and well adult care.
  • Discriminates between normal and abnormal findings, recognizes early stages of serious physical or emotional problems and determines when to refer the patient to the physician for further evaluation and supervision. Makes decisions involving health needs of patients prospectively and collaboratively with the physician. Works collaboratively with the physician in the management of selected complex medical problems.
  • Identifies and manages minor illnesses under established protocols and medical directives. Gives direct physical and psychological care in emergencies and in the presence of illnesses or disabilities to maintain life, provide comfort, reduce stress and enhance coping ability. Identifies where appropriate components of the health regimen may be delegated to nursing personnel and provides teaching and guidance when necessary.
  • Assists in studies relating to the health care and nursing needs of designated groups of people. Assists in planning, instituting, evaluating and revising health care programs as a peer member of the health care team. May be asked to participate in educational research activities.
  • Submits required documentation in a timely manner (credentialing requirements, license renewals, certifications, CNE attendance, etc.). Responsible for their own safety as well as the safety of others. Attends work on a regular and predictable schedule in accordance with clinic leave policy and performs other duties assigned.
Qualifications
KNOWLEDGE, SKILLS, AND ABILITIES: (use of equipment, job related knowledge, language, etc.)
  • Knowledge of PCMH Model
  • Knowledge of nursing techniques and procedures, of the laws and ordinances governing public health and hospital operations.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis, and treatment in the area of specialization.
  • Knowledge of SCF policies and procedures.
  • Knowledge in providing effective Customer Service on a daily basis.
  • Knowledge of computer software (Microsoft Word, Excel, etc.) programs.
  • Knowledge in the use of electronic mail.
  • Skilled in the use of a computer.
  • Ability to provide patient care in the area of specialization, to prepare and maintain records, to organize, coordinate, and evaluate nursing activities and to communicate effectively.
  • Ability to read, write and spell accurately.
  • Ability to maintain an effective and professional working relationship with the public and co-workers.
  • Able to bend, stretch, stoop, stand, sit, and lift.
  • Able to lift 20 lbs.
  • Able to operate a keyboard, telephone, and other office equipment.
  • Ability to examine documents for accuracy and completeness.
  • Ability to prepare records in accordance to detailed instructions.
  • Ability to recognize, evaluate, solve problems and correct errors.
  • Ability to communicate in English and Spanish.

  • Ability to fully comply with the enhanced infection control requirements of the clinic.
MINIMUM QUALIFICATIONS/CERTIFICATES/LICENSES/REGISTRATIONS REQUIRED:
  • Graduation from an accredited four-year college or university.
  • Must be a registered nurse in the State of Texas.
  • Must be Board licensed to practice as an advanced practice nurse.
  • Experience in working with the public.
  • Must be current with BLS/CPR.
  • This position requires an initial N95 mask fit testing and annual test fitting thereafter unless there is a significant change during the year that requires retesting prior to the annual fit testing.

Due to the nature of SC Services, it may be necessary for employees to work extended hours or other variations of the usual shift to ensure adequate care to patients and to maintain service to the community.
