Perform routine examinations, diagnostic testing, and patient follow-up to support positive health outcomes. Manage chronic health conditions and apply quality measurement principles to patient care. Coordinate with healthcare professionals to maintain comprehensive plans of care. Encourage patient participation in health screenings, behavioral health, wellness, and chronic condition management. Support clinic operations, including opening and closing the clinic, inventory management, insurance verification, and maintaining a clean environment.

Critical Information

Patient-facing role; onsite as needed. Approximately 20 hours per week with weekend availability required. May require an on-call rotation; indicate participation in the candidate submittal.
