Job summary

Qualifications

• Board-certified NP licensed in New Jersey.
• Home Health experience preferred.
• Experience conducting in-home Health Risk Assessments is a plus.

• Review medical history and social determinants to enhance care.
• Bridge care gaps in a single home visit.
