You will conduct comprehensive assessments for Medicare Advantage patients during home visits, earning daily compensation ranging from $660 to $990, with mileage reimbursement and a $25 no-show payment; flexible hours, 8am-8pm coverage, and coverage across three counties.

Qualifications

• Board-certified NP licensed in New Jersey.
• 1+ year of Home Health experience.
• Experience conducting in-home Health Risk Assessments.

Responsibilities

• Conduct comprehensive in-home Health Risk Assessments for Medicare Advantage patients.
• Review medical history and social environment to inform care planning.
• Provide insights to bridge gaps in care during a single visit.
