1 Care Partners seeks a compassionate Nurse Practitioner (NP) to provide high-quality care at a Skilled Nursing Facility in Richmond, VA 23229. You will collaborate with physicians and inter-disciplinary teams to assess and manage geriatric patients, develop treatment plans, and promote prevention and continuity of care.

Formación

• Active Nurse Practitioner license in Virginia.
• Experience in a Skilled Nursing Facility (SNF) or geriatric setting preferred.
• Strong clinical assessment and decision-making skills.
• Ability to work independently while collaborating with multidisciplinary team.

Responsabilidades

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Diagnose and manage acute and chronic conditions in coordination with physicians.
• Educate patients, families, and facility staff on disease management and preventive care.
• Maintain accurate and timely documentation in compliance with regulatory guidelines.
• Collaborate with interdisciplinary teams to ensure quality patient outcomes.
