Qualifications

• Minimum 1 year experience as a Family Nurse Practitioner.
• Completion of an accredited Nurse Practitioner program.
• Current, unrestricted Arizona APRN license as a FNP with full prescribing authority.
• Board Certification as a Family Nurse Practitioner (AANP or ANCC).
• Strong communication, collaboration, and adaptability skills.
• Must have Fingerprint Clearance Card or able to obtain.
• Reliable transportation and valid driver's license with proof of auto insurance.
• BLS certified or able to be trained within 5 business days.
• Experience in community health or working with underserved populations preferred.

Responsibilities

• Provide comprehensive primary care across the lifespan, including assessment, diagnosis, treatment, and follow-up.
• Prescribe medications and manage treatment plans within scope and authority.
• Document care accurately in the EHR.
• Collaborate with physicians, nurses, behavioral health providers, and the clinical team.
• Deliver a friendly, professional patient experience and public-facing presence.
• Support clinic workflows and contribute ideas to improve care delivery and efficiency.
• Maintain compliance with organizational policies and state/federal requirements.

We are seeking a compassionate, motivated Family Nurse Practitioner (FNP) to provide high-quality, patient-centered primary care to individuals and families across the lifespan. Key Responsibilities

• Provide comprehensive primary care services across the lifespan, including assessment, diagnosis, treatment, and follow-up care
• Prescribe medications and manage treatment plans within scope and full prescribing authority
• Document care accurately and timely in the electronic health record (EHR)
• Collaborate with physicians, nurses, behavioral health providers, and other clinical team members to support quality patient outcomes
• Deliver exceptional patient experience by consistently maintaining a friendly, professional, and public-facing presence
• Support clinic workflows and contribute ideas to improve care delivery, efficiency, and patient satisfaction
• Maintain compliance with organizational policies and applicable state/federal requirements

Required Qualifications

• Minimum 1 year of experience practicing as a Family Nurse Practitioner (relevant prior medical/clinical experience considered)
• Completion of an accredited Nurse Practitioner program
• Current, unrestricted Arizona APRN license as a Certified Nurse Practitioner (CNP) with a Family/Individual Across the Lifespan focus and full prescribing authority
• Board Certified as a Family Nurse Practitioner (AANP or ANCC)
• Initiative, creativity, problem-solving ability, adaptability, and flexibility
• Ability to collaborate with professional colleagues to provide quality care
• Consistently present a friendly, welcoming, and professional public-facing presence
• Strong verbal, written, and communication skills
• Knowledge of Windows environment and/or Electronic Health Records (EHR) systems
• Ability to provide culturally sensitive services
• Must have or be able to obtain a valid Fingerprint Clearance Card
• Must possess reliable transportation, a valid driver’s license, and proof of auto insurance
• BLS certified or ability to be trained within 5 business days of hire date
• Experience working in community health, integrated care, or with diverse/underserved populations preferred
