Job Description

VillageMD is seeking a Nurse Practitioner to join our home-based primary care team supporting Medicare and Medicare Advantage patients in Griffin, GA. Travel will be required to nearby cities including Lithonia, McDonough, and Monticello. We are growing and looking for providers passionate about excellence and positive change.

Personal and professional skills for success:

• Graduate and certified from an accredited Nurse Practitioner program
• At least two years of home care clinical experience as a Nurse Practitioner or Registered Nurse, with ability to work autonomously
• Comfortable treating adult patients, conducting health histories and physical assessments for acute and chronic conditions
• Knowledge of chronic care management and medication options
• Current, unrestricted State license or eligibility for licensure, and a valid driver’s license
• Active DEA registration or eligibility
• Active Medicare/Medicaid enrollment or eligibility to enroll
• Certification in BLS or ACLS
• Familiarity with EMR systems, preferably Athena, and Microsoft Office
