Job summary

Mass General Brigham is seeking a full-time Nurse Practitioner for a Family Medicine practice in South Deerfield, MA. The NP will provide comprehensive primary care to a panel of about 1,000 patients across ambulatory and inpatient settings.

Responsibilities include direct patient care, exams, orders and interpretation of tests, care plans, documentation, bedside procedures, and prescription management. 40 hours/week with mixed direct care and admin time.

Qualifications

• Master's degree in Nursing required.
• Experience as a Nurse Practitioner 0-1 year preferred; 2+ years in primary care preferred.

• Performs complete histories and physical examinations.
• Orders, interprets, and evaluates laboratory and diagnostic tests.

We are seeking a full-time Nurse Practitioner (40 hours per week) to join our Family Medicine practice at 21 Elm Street in South Deerfield, MA. Our hours of operation are Monday-Friday, 8:00am-5:00pm, with a weekly schedule of 36 hours of direct patient care and 4 hours of administrative time, designated on Fridays.

Our South Deerfield office is a close-knit, community-centered practice dedicated to comprehensive primary care for patients of all ages. We provide physical exams, routine screenings, preventive care, and the diagnosis and management of acute and chronic conditions-with a strong focus on prevention and long-term wellness. Primary care is about relationships, and we take pride in truly knowing our patients to deliver exceptional, personalized care. This role will support a patient panel of approximately 1,000 patients.

Job Summary

The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations.

• Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
• Performs bedside procedures as are appropriate to the patient population.
• Orders medications and writes prescriptions in accordance with organizational and regulatory policies and procedures.
