Central City Health in Detroit is seeking a Nurse Practitioner–level Peer Support Specialist to join our integrated care team. This in-person role focuses on supporting clients with mental illness and housing needs, fostering recovery, coping skills, and independence.

You will provide peer support, advocate for clients, coordinate with clinicians, and help navigate community resources. Required: Master’s in nursing, Michigan NP licensure, BLS certification; 1–2 years in FQHC or community health;

Qualifications

  • Master’s degree in nursing is required.
  • MI Nurse Practitioner licensure is required.
  • 1–2 years of nursing experience in a FQHC, community mental health, public health or primary care setting preferred.
  • Experience with patients with severe and persistent mental illness preferred.
  • Basic Life Support certification required; ACLS preferred.
  • Must be able to work cooperatively with staff and others delivering services to patients.

Responsibilities

  • Provide peer support services to clients experiencing mental illness and homelessness.
  • Act as a patient advocate and provide information and support.
  • Assist patients in regaining independence and developing daily living skills.
  • Collaborate with the integrated care team to coordinate care and resources.

Central City Health is committed to enabling every patient in our community to thrive by building a collaborative multidisciplinary healthcare team. The Peer Support Specialist provides peer support services to clients experiencing mental illness, chronic health condition(s) and/or homelessness. The Peer Support Specialist is an active member of the integrated care team and functions as a role model to peers; exhibiting competency in personal recovery and the use of coping skills; serving as a patient advocate and providing information and support. He/She performs a wide range of tasks to assist patients in regaining independence within the community and mastery over their own recovery process. Through skill building and other supportive interventions, the Peer Support Specialist can assist fellow consumers/patients in effectively utilizing resources, managing symptoms of their illness, and developing daily living skills to gain greater independence.

CCH is expanding our programming, and this may be a great opportunity for you to join our team. CCH participates in the NHSC Loan Repayment Program AND the Public Service Loan Forgiveness Program (PSLF).

Education & Experience:
  • Master’s degree in nursing required.
  • Valid Michigan licensure as a Certified Nurse Practitioner required.
  • 1 to 2 years of nursing experience in a FQHC, community mental health, public health or primary care setting preferred.
  • Experience working with patients exhibiting severe and persistent mental illness preferred.
  • Basic Life Support certification required; ACLS preferred.
  • Must be able to work cooperatively with staff and others involved in the delivery of service to patients.
Job Type:

Full-time.

Pay:

$60.00 per hour.
