Job summary

WellBe Senior Medical is seeking a Nurse Practitioner (APP) to join our field-based home visit team in Charleston, WV. You will deliver comprehensive primary care in patients' homes, manage chronic diseases, and partner with an interdisciplinary care team to optimize outcomes.

This role emphasizes autonomy, compassionate care, and reducing ER visits while providing full benefits, mileage allowance, and relocation support for eligible candidates.

Qualifications

• Advanced degree required: Master's Degree (NP) with focus on Adult, Geriatric, and/or Family.
• WV Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable mobility: Valid driver’s license and vehicle for local field-based travel.

Responsibilities

• Deliver field-based primary care visits and physical exams in the patient's home.

Job Description

Your Impact:
