Job summary

WellBe Senior Medical is seeking a Nurse Practitioner (AP P) to deliver comprehensive primary care in patients’ homes across Northeast Ohio. You will lead field-based visits, develop chronic care plans, and coordinate with an interdisciplinary team to ensure safe transitions and high-quality outcomes.

Qualifications

• Advanced degree required for NP practice.
• OH licensure and board certification required (ANCC or AANP).
• Valid driver’s license and vehicle for local field travel.

Responsibilities

• Deliver field-based primary care visits and physical exams at patients’ homes.
• Create longitudinal care plans to manage chronic disease in high-risk patients.
• Collaborate with interdisciplinary team to ensure safe care transitions across settings.
• Provide in-home management to prevent hospital ER visits.

Your Impact

What You Bring

• Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
• OH Required Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
