Job summary

UnitedHealth Group is seeking a Nurse Practitioner or Physician Assistant to perform in-home health assessments for Medicare Advantage members in Yakima, WA. This field-based role focuses on preventive care and does not include prescribing medications.

You will assess patients, identify care gaps, educate them, and coordinate with primary care providers to improve health outcomes while working a flexible 24-40 hours per week schedule.

Qualifications

• NP requires ANCC or AANP certification.
• PA requires NCCPA certification and state licensure.
• Active, unrestricted license is required.
• Valid driver's license and reliable transportation are necessary.
