Catholic Health in Smithtown, NY is seeking a Nurse Practitioner to provide direct and collaborative care for sub-acute, rehabilitation and long-term care residents in our St. Catherine of Siena campus.

You will perform exams, assessments, diagnoses, prescribe treatments, and work with the Medical Director and nursing staff to ensure timely care and regulatory compliance. A Master’s in NP, NY NP license and BLS/ACLS are required; experience in acute or long-term care is preferred and

Responsibilities

  • Coordinates with Medical Director, attending physicians, nursing and rehab staff to plan sub-acute, rehabilitation and long-term care residents’ treatment.
  • Performs physical examinations, assessments, medical diagnoses and prescribes treatment modalities for residents.
  • Ensures timely initiation, following and evaluation of tests, therapies and medications.
  • Collaborates to monitor policies, procedures and nursing standards of care for regulatory compliance.
  • Provides resident, family and staff education and participates in performance improvement.
  • Demonstrates culturally and religiously sensitive care and respect for patient rights.

St. Catherine of Siena Nursing and Rehabilitation and St. Catherine of Siena Hospital are proud members of Catholic Health and have served the residents of the Smithtown area for more than 50 years. Certifications & Qualifications
  • Licensed by the New York State Department of Health
  • Certified by the Medicare and Medicaid Program
  • Operated by a New York State Licensed Nursing Home Administrator
  • Graduate of a Master’s in NP program
  • NP License and NP Board Certification
  • BLS and ACLS certification
  • Prior experience in acute, sub‑acute, rehabilitation and long‑term care preferred
Duties
  • Coordinates with the Medical Director, attending physicians, nursing staff, rehabilitation staff and ancillary support staff in providing the direct and collaborative treatment plan for sub-acute, rehabilitation and long-term care residents.
  • Performs physical examinations, comprehensive patient assessments, medical diagnoses and prescribes appropriate treatment modalities for this resident population.
  • Ensures that all tests, therapies, medications and treatments are initiated, followed and evaluated in a timely manner.
  • Collaborates with the Director of Nursing to monitor that the Nursing Home’s policies, procedures and nursing standards of care are appropriately followed and maintained, and reviews/audits nursing assessment, planning, intervention, evaluation and medical record documentation for regulatory compliance.
  • Provides resident, family and staff education and participates in performance improvement.
  • Demonstrates care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights.
Salary Range

USD $144,664.00 – USD $187,200.00 / Yr. This range serves as a good faith estimate; actual pay will encompass a number of factors, including the candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses, incentives, differential pay or other forms of compensation that may be applicable to this job, nor does it include the value of benefits.
