Premise Health Inc. is hiring a Full-Time Occupational Health Nurse Practitioner to provide onsite care for client employees at our Swiftwater Health Center in Swiftwater, PA.

The role offers a 32-hour week, with a Monday, Wednesday, Thursday and Friday schedule from 8:30 AM to 5:00 PM, and no nights or weekends. You will conduct physical exams, perform injections and wound care, develop care plans, and coordinate with physicians and client teams on health promotion and safety activities.

Qualificações

• Requires RN and NP licensure and national NP certification.
• Prescriptive authority and DEA license required where applicable.
• BLS/ACLS/occupational health certifications preferred.

As a Full-Time Occupational Health Nurse Practitioner, you’ll provide care to client employees at our Health Center in Swiftwater, PA. This role offers a consistent, 32 hours per week, schedule of Monday, Wednesday, Thursday, and Friday, 8:30 AM–5:00 PM, with no nights or weekends.

What You’ll Do

• Conducts physical exams and preventive health measures per guidelines
• Performs procedures such as injections, suturing, wound care, and infection management
• Develops patient care plans, records progress, and ensures continuity of care
• Develops patient care plans, records progress, and ensures continuity of care.
• Educates patients on treatment plans, health maintenance, and development.
• Orders and interprets diagnostic tests to assess clinical conditions.
• Collaborates with physicians and healthcare teams for comprehensive care.
• Collaborates with Medical director and client stakeholders on Health Promotion programs, surveillance needs, ergonomics, process development, and other related safety/injury prevention opportunities.
• Refers patients to specialists or physicians as needed.
• Maintains patient health records ensure complete, accurate and up-to-date documentation and records.
• Educates and coaches nursing staff on best practices.
• Performs other duties as assigned.

What You’ll Bring

• Maintains state RN licenses/certificates to practice.
• Maintains NP licenses/certificates to practice.
• Maintains State Prescriptive Authority (if applicable) .
• Federal DEA license (matching each state of practice as applicable) .
• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration).
• Current national certification (ANCC or AANP required).
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required).
• Current certification with hands‑on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services.
• Certification in occupational health (COHN/COHN‑S/COHN‑S/CM) preferred.
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting.
• Experience working in an occupational health setting is preferred.
• Experience performing site visits and working with safety teams establishing ways to mitigate workplace hazards.
