Job summary

Carease Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to deliver preventive care to residents in skilled nursing facilities. The role combines on-site visits with remote work from home to support remote monitoring and chronic care management.

You will collaborate with multidisciplinary teams, maintain accurate charting with a dedicated scribe team, and stay current with geriatric and internal medicine best practices to improve patient outcomes.

Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of experience in Geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.
• Ability to work independently and as part of a team.
• Strong problem-solving and critical thinking skills.

Responsibilities

• Provide preventative care to residents in skilled nursing facilities, including conducting on-site visits and remote monitoring of vital signs.
• Collaborate with healthcare teams to develop and implement individualized care plans, while ensuring continuity of care and coordination with other healthcare providers.
• Conduct documentation and charting in a timely and accurate manner, with the support of a dedicated scribe team.
• Participate in multidisciplinary team meetings and clinical decision-making, utilizing your expertise in Geriatrics and Internal Medicine.
• Build strong relationships with facility staff, patients, and families, while maintaining a patient-centered approach.
• Stay current with industry developments and best practices through ongoing education and training.
