Job summary

Carease Health in Texas is seeking an experienced Nurse Practitioner (NP) with a geriatrics or internal medicine background to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

You will build strong relationships with facilities, maintain a patient-centered approach, and collaborate with a multidisciplinary team.

Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of geriatric care experience.
• Experience with documentation review and collaborative care.
