Job summary

NPA seeks a Nurse Practitioner to independently manage 90-100 patients across multiple nursing and adult home facilities. You will focus on clinical management, proactive care, and preventing hospitalizations, while collaborating with social workers and RNs to deliver comprehensive care.

The NP sets their own schedule, conducts routine visits, acute assessments, and post-hospitalization follow-ups, with a Monday–Friday 8–5 schedule and on-call duties as needed.

Qualifications

• Active NYS NP license with board certification.
• Strong clinical judgment and ability to identify health changes.
• Effective written and verbal communication with patients, caregivers, and staff.
• Proficient with EHR systems (MedEnt a plus).
• Ability to work independently while maintaining facility relationships.
• Maintain NYS licensure, board certification, and CPR certification; travel to facilities with own vehicle.

Job description

Reporting to the I-SNP Team Lead, the Nurse Practitioner will independently manage a caseload of 90-100 patients across multiple nursing and adult home facilities, focusing on clinical management and proactive care to prevent hospitalizations. This role emphasizes treating in place, collaborating with social workers and RNs to deliver comprehensive care. The NP sets their own schedule, conducting routine visits, acute assessments, and post-hospitalization follow-ups while fostering strong relationships with facility staff. The position follows a Monday-Friday, 8-5 schedule, with some on-call responsibilities as needed. FCNY offers a team-oriented culture with strong retention and career longevity.

• Active Nurse Practitioner (NP) license in New York State with board certification.
• Strong clinical judgment and ability to identify subtle changes in patient health.
• Effective written and verbal communication skills for working with patients, caregivers, and facility staff.
• Basic computer skills and proficiency in electronic health records (EHR) (MedEnt experience a plus).
• Ability to work independently while maintaining strong facility relationships.
• Maintain active NYS licensure, board certification, CPR certification, and provide own vehicle with auto insurance for travel to facilities.
