Job summary

United Health Services, Inc. is seeking a Geriatric Nurse Practitioner to provide comprehensive health care within our local nursing facilities.

You will document, order and interpret diagnostics, prescribe and monitor therapies, and develop patient management plans with thorough electronic medical record entries. Qualifications

• MSN degree is required.
• NPI number is required; evidence by notification.
• Successful completion of an approved Nurse Practitioner program.
• Three years of professional nursing experience in a clinical setting.
• Current NYS Registered Professional Nurse License.
• ANCC or AANP certification.
• Current NYS Certified Nurse Practitioner license/certification.
• CPR certification within one week of hire and maintained during employment.
• Certified Prescriptive Privilege in NYS.

Responsibilities

• Provide health care services to patients within local nursing facilities.

Job description

The Geriatric Nurse Practitioner will provide health care services to patients within our local nursing facilities. They will complete appropriate documentation, administer and order diagnostic tests, interpret the test results, and will prescribe and monitor therapies. They will develop and implement patient management plans, electronic medical records progress notes, and assist in providing continuity of care which includes instruction and educating patients regarding compliance with prescribed therapeutic regimens and health maintenance.

This position may require some local travel between nursing facilities. The Nurse Practitioner will also be part of an on–call group with support from Geriatric physicians and other nurse practitioners.

Education/Experience

Minimum Required:

• Master of Science in Nursing (MSN) degree
• Possession of an NPI (National Provider Identification) number, as evidenced by the receipt of a copy of the notification assigning the number
• Successful completion of an approved Nurse Practitioner program
• Three (3) years of professional nursing experience in a clinical setting

License/Certification

Minimum Required:

• Current NYS Registered Professional Nurse License
• American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) certification
• Current NYS Certified Nurse Practitioner (CNP) license and certification with New York State Education Department
• Documentation of current CPR certification required within one (1) week of hire, with maintenance of current CPR certification during employment
• Certified Prescriptive Privilege in NYS

Salary Range: $137,500 – $160,400 annually

Work Environment

• All work is performed in offices and patient examination rooms that require working in close quarters with other employees and patients. The offices are air conditioned/heated. Travel to other UHS Physician Practice locations may be necessary.
• PPE is used as required by Infection Control policies.
