Pine Park Health is seeking a Full-Time Nurse Practitioner to deliver care in the San Diego market (Escondido, Carlsbad, Vista). You will manage a smaller panel of complex patients within a value-based care model and collaborate with a multidisciplinary team to provide high-quality patient care.

The role requires onsite care across the Northern San Diego region, strong communication, and the ability to work in a dynamic environment. A focus on holistic, patient-centered care is essential.

Responsibilities

• Provide onsite primary care to older adults living in Senior Living Communities.
• Develop and execute detailed person-centered care plans for all primary care patients.
• Work with Medical Director to identify and develop policies affecting daily practice and clinical guidelines.
• Provide monthly Chronic Care Management (CCM) visits and sign off CCM documentation.
• Lead regular huddles and provide leadership to the clinical team.
• Coordinate with senior living management and care teams to improve Pine Park care plans and patient outcomes.

The Opportunity:

Pine Park Health is looking for a Full-Time Nurse Practitioner responsible for delivering care in our San Diego market (Escondido, Carlsbad, Vista). If you are excited about the opportunity to work with a smaller patient panel of complex care patients, contribute to a value-based care model, and work with a motivated multi-disciplinary team to provide high-quality patient care you have come to the right place. This person-centered care model works to leverage the power of our care navigators to maximize the time and energy of our providers.

What you'll do:

• Provide onsiteprimarycareservices to older adults living in Senior Living Communities


• Develop and execute detailedperson-centered careplans on allprimarycarepatients


• Work with Medical Director to identify and develop policies to address issues thatarise which affect day to daypractice, culture or clinical guidelines, including for emerging disease(s)


• Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentation


• Run regular huddles and provide leadership to the clinical team


• Identify ways of working withsenior living management andcareteams to better carry out Pine Parkcareplans and improve patient outcomes

Qualifications:

• Nurse Practitioner - preferably with geriatrics experience


• Comfortable with a fast-paced and changing environment


• Willingness to provide onsite clinical across the Northern San Diego region


• Desire to work with complex care patients and support the building of a holistic care model


• Strong orientation towards collaboration and working closely with a multidisciplinary team; values input of all members of the clinical team


• Strong written and verbal communicator


• Ability to work with tech-enabled care delivery tools (e.g.,project management, 'checklist' management, remote monitoring, etc.) used together to improve patientcareand team function


• Flexible work schedule with travel within the Northern San Diego area. This role involves driving between partner communities as needed

The base salary range for this role is $135,000 to $150,000 plus generous bonus, equity, and benefits. The final compensation package for each successful candidate will depend on several job-related factors unique to each candidate. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data. Our compensation structures and ranges are tailored to each geographic zone's unique market conditions to ensure all employees receive fair and competitive compensation based on their roles and locations.
