Job summary

Your Talent Team is seeking a full-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care in a community health center setting in the Greater Sacramento area of CA. The role offers relocation assistance and a schedule of 8–5, or a 4x10 option, within a supportive multidisciplinary team.

Qualifications include active CA RN and NP licenses, NP board certification, DEA and BLS, with 1–2 years of relevant experience.

Qualifications

• Active California RN and Nurse Practitioner license.
• Current Nurse Practitioner board certification.
• Current DEA certification.
• Current BLS certification.
• 1-2 years of clinical experience preferred, especially in community health.
• Commitment to serving low-income, homeless, or underserved communities.
• Comfortable treating three patients per hour.
• EPIC experience.
• In-basket management competency.
• Bilingual in English and Russian, Farsi, Pashto/Pushto, or Spanish preferred.

Responsibilities

• Perform comprehensive patient evaluations, including history and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and interventions within scope of practice.
• Develop and implement treatment plans with Medical Director.
• Monitor patient progress and adjust care plans as needed.
• Initiate emergency procedures when indicated.
• Provide preventive care, health education, and counseling.
• Maintain accurate documentation per agency and regulatory standards.
• Collaborate with physicians and other healthcare professionals for coordinated care.

Compensation: $131,000 - $170,000 annually (based on experience)

Job Type: Full-time, Mondays to Fridays 8-5 PM or 4 day (10 hour shift available).

Location: Greater Sacramento area, CA (Multiple locations available)

Key Responsibilities

• Perform comprehensive patient evaluations, including history and physical examinations

• Order and interpret laboratory tests, imaging, and other diagnostics
• Prescribe medications and therapeutic interventions within scope of practice
• Develop and implement treatment plans in collaboration with the Medical Director

• Initiate emergency procedures when clinically indicated
• Provide preventive care, health education, and counseling services
• Maintain accurate and timely documentation in accordance with agency and regulatory standards
