Job summary

Liberty Hospice in Roanoke Rapids, NC, seeks an experienced Hospice & Palliative Care Nurse Practitioner to implement palliative care program requirements and conduct patient visits in accordance with guidelines.

The ideal candidate holds a Master’s degree in Nursing, an active NC NP license, and a certification as an Adult or Family NP. Hospice care experience is preferred, with excellent communication and organizational skills to coordinate care across the team.

Qualifications

• Master's degree in Nursing is required and active licensure in the state of practice.
• Certification as an Adult or Family Nurse Practitioner is required.
• Hospice or Palliative Care nursing experience is preferred.
• Knowledge of the hospice care program structure and operations.
• Excellent communication and organizational skills.

Responsibilities

• Implements palliative care program requirements.
• Completes medical visits with patients per guidelines and coordinates care with the Palliative Care team.
• Conducts face-to-face encounters to provide re-certification information to the Medical Director/Associate Medical Director for Hospice Patients.
• Communicates clinical findings to Primary care physicians and supervising Medical Directors.
• Communicates with patients/families and with all agency personnel.

HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER

Job Description:

• Implements palliative care program requirements


• Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated


• Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients


• Provide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical Director


• Communicate with patients/families at all levels.


• Communicate with all agency personnel

Job Requirements:

• Master's degree in Nursing required, with current licensure in the state of practice


• Certification as an Adult or Family Nurse Practitioner


• Experience in Hospice or Palliative Care nursing preferred


• Knowledge of the framework, function, and organization of a hospice care program.


• Excellent communication and organization skills.
