Job summary

Liberty Hospice in Wilmington, NC is seeking an experienced Hospice Licensed Practical Nurse (LPN) to join our full-time team. You will provide direct patient care, document care plans, and coordinate hospice services under the supervision of the Registered Nurse.

The ideal candidate holds an active LPN license in North Carolina, possesses CPR certification, and has at least one year of clinical nursing experience, preferably in acute care settings.

Qualifications

• Nursing education from an approved school.
• Current LPN licensure in the state with one year of clinical nursing experience.
• Ability to independently organize work and provide effective hospice care.
• Ability to thoroughly and timely document patient care.
• Strong oral and written communication skills.
• CPR certification.

Responsibilities

• Provides direct patient care per the plan of care.
• Completes documentation as required by policy and regulation.
• Follows the plan of care developed by the Registered Nurse.
• Coordinates hospice services with the RN, Patient Care Coordinator and other personnel to optimize care.
• Transcribes physician orders and forwards notes to the RN.
• Attends and participates in IDT meetings.

Job Description

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates Hospice services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Hospice Registered Nurse for coordination of care.
• Attends and participates in IDT meetings.

Job Requirements

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to hospice patients.
• Ability to ensure appropriate patient care as supervised by the Hospice Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
