Job summary

Quadrant, Inc. is seeking a Nurse Practitioner for Concierge Medicine in Reston, VA. The role offers an hourly pay starting at $68 and a pathway to a robust annual compensation.

The NP will deliver comprehensive primary care, wellness visits, and health risk assessments, while coordinating care with multidisciplinary teams and educating patients on conditions and treatment plans. Requirements include MSN/DNP/NP certificate, National FNP certification, active NP license, and active DEA license.

Qualifications

• Experience in primary care (internal medicine, geriatrics, or family medicine).
• Familiar with HCC, ICD-10 coding, and HRAs.

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments with thorough documentation.
• Coordinate care with other providers, testing facilities, and agencies.
• Lead population health in coordination with the Care Team.
• Assist the care team with phone triage and outreach.
• Educate patients on health conditions and care plans.

Nurse Practitioner - Concierge Medicine

Reston, VA

Pay From: $68 an hour

MUST:

• Experience in primary care: internal medicine, geriatrics, or family medicine
• Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs)
• Passion for teamwork and the opportunity to collaborate cross-functionally
• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification as a Family Nurse Practitioner
• Active, non-probationary state Nurse Practitioner license
• Active DEA license

DUTIES:

• Provision of exceptional primary care
• Conduct office visits for routine and acute issues
• Administer annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation
• Care coordination with other providers, specialists, testing facilities, and agencies
• Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years)
• Assisting the care team with phone triage and outreach
• Educating patients on their health conditions, care plans, and treatments
