Job summary

Optum Home & Community Care in Virginia is seeking an Advanced Practice Clinician (NP/PA) to join the Senior Community Care team serving long-term care, senior housing and home care settings. The role includes comprehensive assessments, care planning, documentation, and collaboration with the care team.

Compensation ranges from $109,500 to $164,000 with a no on-call, no weekends, and no holidays schedule; a $20,000 sign-on bonus is available for external candidates, plus a comprehensive benefits

Qualifications

  • Active ANCC or AANP certification for NP, or NCCPA certification for PA, and current licensure.
  • Active, unrestricted license in the practicing state.
  • DEA license or prescriptive authority, or ability to obtain post-hire.
  • Active driver's license with reliable transportation.
  • Ability to gain a collaborative practice agreement as required.

Responsibilities

  • Conduct comprehensive assessments including history, vitals, and exam.
  • Support members during short-term sub-acute rehab.
  • Ensure complete documentation including ICD-10.
  • Collaborate with interdisciplinary care team.
  • Conduct advanced illness and end-of-life conversations.
  • Provide counseling and education to patients and caregivers.

The Senior Community Care (SCC) program is an integrated care delivery program that coordinates the delivery and provision of clinical care of members. The Advanced Practice Clinician within SCC provides care to our highest-risk health plan members. Primary Responsibilities:
  • Conduct comprehensive assessments, including:
    • Past medical history review, medication reconciliation, vital signs, and physical exam
    • Evidence-based screenings and point-of-care testing (as appropriate)
  • Support members during a short term sub-acute rehabilitation episode
  • Ensure accurate and complete documentation, including ICD 10 conditions
  • Communicate and collaborate with the interdisciplinary care team
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education

Required Qualifications:
  • For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • For PAs: Current NCCPA certification and state licensure
  • Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
  • Active and unrestricted driver’s license
    • In addition to a driver’s license, access to reliable transportation
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to navigate varied environments and to position oneself as needed to perform job duties
Preferred Qualifications:
  • 1+ years of clinical experience as an APC (long-term care setting preferred)
  • Experience in geriatric medicine, long-term care, senior living or home care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficiency with electronic medical records and technology

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
