Health Solutions in Southern Colorado is seeking a full-time Family Nurse Practitioner for the Perry Hall Family Medicine site. You will provide comprehensive primary care across adults and children, coordinating with a multidisciplinary team to ensure patient-centered, preventive care.

The role emphasizes evidence-based practice, quality measures, and collaboration with physicians, nurses, and behavioral health staff to meet PCMH standards.

Qualifications

  • Licensed as an Advanced Practice Nurse in Colorado or a compact state.
  • Current D.E.A. license.
  • CPR certification with ACLS within 3–12 months if appropriate.
  • Minimum 5 years RN experience and 3 years as a Family Nurse Practitioner.
  • Must be insurable under company malpractice coverage.

Responsibilities

  • Deliver comprehensive primary care across the lifespan, including health assessments and chronic disease management.
  • Order, perform, and interpret diagnostic tests to support diagnoses.

With over 400 employees in Southern Colorado, you would be joining a mighty team of support and administrative staff, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community.We’re expanding and hiring a full-time Primary Care Family Nurse Practitioner to work at our new Health Solutions Family Medicine- Perry Hall location.This position is based at a HRSA-certified healthcare site recognized for meeting rigorous federal standards and expanding access to comprehensive patient care.This position is eligible for a sign-on bonus: $5,000 upon successful completion of orientation; $2,500 at successful completion of 1 year of employment; $2,500 at successful completion of 2 years of employment.What You’ll Be DoingAs a provider the full scope of primary care services which fall under primary care of adult and pediatric populations, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance for this population. In addition, a Family Nurse Practitioner (FNP) is a member of the Health Solutions Medical Services Team. In that capacity they are expected to be familiar with the philosophy, goals and objectives of the Medical Center and work collaboratively to achieve them. They must be cognizant of, and comply with, all Center policies and procedures, as well as pertinent state and federal regulations.RESPONSIBILITIESAs a Family Nurse Practitioner (FNP) with Health Solutions Family Medicine- Perry Hall, you will play a central role in delivering integrated, patient-centered care aligned with PCMH principles and APM performance metrics. Under the supervision of the Director of Medical Services and Vice President of Medical Services Officer, your responsibilities include:Clinical CareProvide comprehensive primary care across the lifespan, including health assessments, physical exams, diagnostic evaluations, and management of acute and chronic conditions using evidence-based guidelines.Order, perform, and interpret diagnostic tests (e.g., blood work, imaging, EKGs, urinalysis) to support accurate diagnoses.Develop and adjust individualized treatment plans, prescribing medications, therapies, and preventive interventions as appropriate.Deliver preventive services such as wellness visits, vaccinations, screenings, and routine check-ups.Engage patients in shared decision-making and self-management support; educate on disease prevention, healthy habits, and self-care strategies.Prepare and administer prescribed medications, injections, and vaccinations.Review lab results with patients and treatment teams; apply best practices in medical care and medication-assisted treatment.Evaluate behavioral health assessments and refer to behavioral health professionals as needed.Perform office-based procedures within scope of practice and participate in acute care situations (e.g., urgent triage).Care Integration & CollaborationFunction collaboratively within a multidisciplinary team of physicians, nurses, behavioral health clinicians, care coordinators, pharmacists, and ancillary staff to ensure continuity of care.Participate in daily huddles, case conferences, and care planning meetings.Coordinate referrals and care transitions across multiple providers and healthcare settings.Quality & ComplianceMaintain accurate, timely electronic health records and documentation supporting medical necessity, quality metrics, and compliant billing; collaborate with billing and quality teams as needed.Deliver care consistent with PCMH principles: whole-person orientation, coordinated care, enhanced access, and continuous quality improvement.Participate in quality initiatives and use data to drive performance on APM measures (e.g., patient engagement, preventive screenings, chronic disease outcomes).Meet productivity, patient access, and panel management benchmarks aligned with organizational and payer requirements.Adhere to confidentiality standards, medical ethics, and Colorado statutes related to collaborative practice and prescriptive authority.Professional DevelopmentStay current with medical research and guidelines through continuing education and integration of evidence-based practices.Advance the education of healthcare team members to promote high-quality care.What You’ll Like About UsCompetitive pay $135,000-$145,000 Annually (Dependent on experience, certification and shift)Generous benefits package.
