Qualifications

• Actively licensed to practice in the state of employment.
• Current DEA License required on start date.
• Current Controlled Substances License required on start date.
• ANCC or AANP certification.
• Desire to work in Physiatry/PM&R.
• Orthopedic experience is a plus.
• Physiatry experience is a plus.
• Skilled Nursing experience is a plus.

Responsibilities

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for rehabilitation patients.
• Conduct thorough patient assessments, including histories, exams, and diagnostics to set rehab goals.
• Monitor progress and adjust treatment plans to improve function and independence.
• Participate in multidisciplinary meetings, case conferences, and educational activities.
• Maintain accurate documentation of evaluations, plans, notes, and discharge summaries in EMR.
• Educate patients and families about rehabilitation processes, outcomes, and post-discharge care.

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs


• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions


• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence


• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills


• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems


• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care

Requirements
